SESSION 2019 - 20 In compliance of the decision of Pakistan Medical Commission passed in the Meeting held on 25th October, 2019 at NHSR&C, Islamabad, LIAQUAT UNIVERSITY OF MEDICAL & HEALTH SCIENCES, Jamshoro being the ADMITTING UNIVERSITY, invites Applications from the Candidates having Domicile of Sindh Province, with Aggregate Score of up to 65% for MBBS and 55% for BDS, from 16th DECEMBER, 2019 to 19th DECEMBER, 2019. ELIGIBILITY: • Candidate must have passed Intermediate / HSSC Pre-Medical Group or Equivalent Examination (e.g. A-Level. 12th Grade etc) with Minimum 60% Marks (As per Sindh High Court Decision). • Candidate MUST have appeared in Entry Test conducted by NTS on 15th September, 2019. PROCEDURE TO APPLY : • Candidate has to download Application Form from the Website of LUMHS (www.lumhs.edu.pk) and shall forward along with all required Documents, Paid Bank Voucher of Rs. 2,500/- (Non-Refundable as Processing Fee) to Directorate of Admissions. LUMHS, Jamshoro up to 19th December, 2019 through TSC. • Incomplete Applications and those submitted after the Last Date will NOT be accepted. • TCS shall charge Rs. 200/- from the Candidate at the time of Submission of Filled Application Forms as Transport Charges. • List of TCS Outlets is available on LUMHS Website: www.lumhs.edu.pk LIST OF REQUIRED DOCUMENTS: 01. Copy of Matriculation Certificate (SSC) or 0-Level Equivalence Certificate by IBCC 05. Copy of Matriculation Marks Certificate or '0' Level Transcript Certificate 06. Copy HSSC Pre-Medical Marks Certificate / A Level Transcript Certificate 07. Copy of HSC Pre-Medical Certificate (Provisional) or A-Level Equivalence Certificate by IBCC 08. National Identity Card from NADRA B' Form of Candidate 09. Domicile and PRC Certificate of Candidate 10. Computerized National Identity Card of Father from NADRA (in case of Death of Father or Divorce, then Mother) 11. Hafiz-e-ouran Certificate (If Applicable) from any Reputable Madrasah 12. Applicants desirous of Admission in PRIVATE MEDICAL and DENTAL INSTITUTION shall submit a Wealth Statement of either Parent / Guardian, Equivalent Certificate to ensure the Payment of Fee for the remaining Duration the Course in case of demise of Parent / Guardian, to the Institution, in which they get the Admission and shall submit Insurance Certificate to ensure the Payment of Fee for the remaining period in case of demise of Parent / Guardian 13.
